All graduating high school students who are residents of the Beaver Dam Unified School District and are planning to attend a four year college or university or a one or two year technical college may apply.

In the POST SECONDARY SCHOOL DATA SECTION, select either a 4-year college, or vo-tech, or two year community college. [Select only one]. The name of the specific post secondary school is required. You may change your choice of the school later.

After the BLUE and WHITE forms have been completed, mail them to the above address.

Have a teacher or guidance counselor complete the YELLOW sheet. Teachers will then forward it to the guidance office for completion of the transcript information. The Guidance Department will forward it to the Scholarship Foundation.

Information on the Financial Assistance Questionnaire (BLUE SHEET) will be held in strict confidence, only to be seen and evaluated by the Scholarship America Foundation in St. Peter, MN.

APPLICATIONS MUST BE POSTMARKED BEFORE OR ON MARCH 2, 2009. Awards will be announced in May.

Anyone already receiving a full tuition, room, and board scholarships is not eligible.

Incomplete applications will not be processed

SPECIAL SCHOLARSHIPS

There are special scholarships available for employees or dependants of the Conley Publishing Co., Apache Stainless and Burgess Norton. Please note where indicated on the application.

There are special scholarships available for students entering the medical, fine arts, business or education fields. Please indicate your course of study preference on the application.

There is a new scholarship for a student majoring in the fine arts.
